I don’t see times on the form, is this confirmed? Appointment request submitted through the form on this page are not confirmed appointments. They are simply an appointment request that we will use to find the best date, and time according to availability and what is convenient for your schedule. 

Can I request a specific day/time? Absolutely, add your preferences for date and time in the form in order for us to accommodate you. 

What should I bring to my appointment? Be sure to bring your ID/Driver’s license if you’re an adult, insurance card if you aren’t a cash pay patient, and a list of medications you’re currently taking.
